Payment is expected at time of appointment.

We are not in network with any insurance companies. We do not accept payment from any insurance companies.

Phone calls over 10 minutes may be billed, depending on the situation.

Review of past reports, materials may be billed depending on the situation.

Missed appointments and/or appointments cancelled with less than 24 hours notice will be charged full fee.
